It is 5 o'clock on a Friday, and your 12 year old trauma patient in intensive care is requiring multiple points of peripheral access for vascular therapies. You have 3 PIVs (peripheral IV) and an arterial line, and one of your PIVs has infiltrated. You know that obtaining a new PIV in this patient is not an option due to their current condition, and the patient's attending has left for the day, leaving your only support to be the resident or fellow covering the night. Does this scenario look familiar? What do you do? This is a common theme across many hospitals leaving the bedside clinician frustrated. Learn more about one hospital's approach to this exact problem through the development of unit-based services and an institutional vascular access pathway. By clearly outlining the route and defining resources available to obtain access, a proactive plan can be developed, thus eliminating these reactive situations.

Carol Czajka has been a nurse for 38 years. She is currently a Clinical Expert with the Vascular Access Team at Children's Hospital of Philadelphia. Clinical , education and administrative responsibilities include insertion and care of peripheral and central venous access and establishing evidenced-based policies, procedures and standards for neonatal and pediatric IV therapy. Carol works closely with the NICU and PICU teams by providing vascular access support in their complicated patient population, assisted in the development of intensive care bedside PICC programs, and works along side their quality improvement team to help educate the staff and reduce harm metrics. Carol is an active participant in hospital wide initiatives for the advancement of vascular access needs and harm reduction. She has presented at several local, regional and national conferences, on topics related to vascular access. Carol is certified by the Pediatric Nurses Society and the Association for Vascular Access.

Thomas Conlon, MD

Thomas Conlon is a Pediatric Critical Care physician at The Children's Hospital of Philadelphia. His academic interests are in educational and clinical outcomes related to diagnostic and procedural ultrasound applications. He has developed fellow and faculty ultrasound curriculum within his institution and leads courses locally, nationally and internationally. He has authored numerous chapters and articles within the scope of pediatric acute care ultrasound. He has also led multi-discipline collaboration to expand vascular access training and infrastructural support within the critical care setting.
