Jodi Hubler is the Managing Director of Lemhi Ventures, a health care services venture capital fund with $335M under management. For over a decade, Lemhi has invested in companies that transform and disrupt health care. Jodi is known as insightful and direct, whether as a strategic advisor or mentor to entrepreneurs and CEOs or as an investor and board member. She focuses on leadership, culture change, business strategy alignment, governance, and operational excellence in Lemhi and its portfolio companies.

Over her 20-plus-year career, she has held executive positions in a variety of public and private companies, including a lead role in global human resource re-engineering efforts for Cargill; driving systems to support lean manufacturing and process change for a $2B division of Alcoa; and start-up experience with venture-backed Definity Health, where she served as chief people officer. Definity Health was ultimately acquired by United Health Group for more than $300M.

Jodi is a graduate of the University of Iowa Tippie College of Business, with a B.A. in Administration and a M.A. in Industrial Relations. Jodi currently serves on the boards of Digital Reasoning, PokitDok, and PlanSource and serves as Chair of Bind and Recondo Technology. Jodi previously served on the boards of Carol, DNA Direct, Destination Rx, Liazon, OneHealth Solutions (Chair), RetraceHealth (Chair) and ShareableInk. She is a frequent panelist at venture capital conferences and is a member of the Healthcare Executives Leadership Network.

Kaveh Safavi

Kaveh Safavi, M.D., J.D. is the senior managing director for Accenture’s global healthcare business. Dr. Safavi is responsible for developing and driving a growth strategy that differentiates Accenture’s offerings for providers, health insurers and public and private health systems across the globe.
Dr. Safavi joined Accenture from Cisco, in 2011, where he led the global healthcare practice. Prior to that, he was Thompson Reuter’s chief medical officer of its Health Businesses and United Healthcare’s vice president of medical affairs. He also has served in leadership roles at HealthSpring and Humana. As a seasoned executive, Dr. Safavi brings more than three decades of leadership experience to Accenture Health including such accomplishments as establishing one of the Midwest’s first electronic health record-enabled primary care practices. He has published numerous papers and is quoted on healthcare issues in various media publications, including the Wall Street Journal, BBC, New York Times, US News and World Report, Harvard Business Review and the Economist.
Dr. Safavi serves as a member of the board of directors for Xcertia, an organization that supports industry guidelines for safe and effective mobile health apps. He’s also serves on the
advisory committee of the Buck Institute for Research on Aging.
Dr. Safavi has earned the distinction of both medical and law degrees, an M.D. from Loyola University School of Medicine and a J.D. from DePaul University College of Law. He is board-certified in internal medicine and pediatrics and completed his residency at the University of Michigan Medical Center. Dr. Safavi is a lifelong Chicagoan. David Smith

Founder
Third Horizon Strategies

David is the founder of Third Horizon Strategies, an organization that supports health care companies in
strategic planning responsive to current and expected policy and market environmental conditions.
Prior to organizing Third Horizon, David was the Chief Client Strategies Officer and a Partner with Leavitt
Partners.
His expertise lies in the areas of managed care, alternative payment models, and public health. He has
organized several coalitions focused on these efforts, most notably in the areas of opioid use disorder
recovery, individual market stabilization, and consumer-engagement platforms. He is also the cofounder
of the Health Care Council of a Chicago, a group of Chicago-based health care businesses
focused on economic development, system transformation, and social disparities.
David serves on the board of the Sinai Hospital System, begins a board appointment with HIMSS North
America in the summer of 2018, and is on the Founder’s Council of United States of Care, a new nonpartisan
initiative focused on expanding health care access.
David lives in Chicago with his wife and three children.

Ann Mond Johnson

Ann Mond Johnson was named Chief Executive Officer of the American Telemedicine Association (ATA) in February 2018. With 25 years of healthcare leadership and entrepreneurial experience, Ann’s background includes launching, building and leading companies that have been innovators in using health care technology and data to improve consumer experience and maximize consumer benefit.

Prior to her appointment as CEO of ATA, Ann served as CEO of Zest Health, a technology-enabled service that enables consumers to optimize their benefits by combining a mobile first solution with access to live support. Under her leadership, Zest solidified its value proposition and doubled its base business in one year. Prior to joining Zest, she served as Board Chair and Advisor to ConnectedHealth, a leading provider of private insurance exchanges. Working closely with the cofounders, she was responsible for strategy and business development as well as evangelizing consumerism and the benefits of exchanges in the media and through industry events.

In 2000, Ann co-founded and served as CEO of Subimo, a pioneer in healthcare cost and quality transparency tools for consumers. Under her leadership, Subimo’s reach extended to major national and regional health plans (including a majority of the Blues plans) as well as Fortune 500 companies. Ann raised start-up funds, led the management team through significant growth and positioned the company as a market leader. Subimo was acquired by WebMD in 2006.

Previously, Ann was Senior Vice President at Sachs Group (now part of IBM Watson/Truven Health), the leading provider of healthcare information for health organizations. She led the product development and operations for the company, providing data and applications for hospital systems, payers and medical device companies. Ann drove the expansion of the company into new markets (including carriers) and the transition from a desktop software company to a
data information company.

Ann’s ability to inspire and lead diverse teams has helped build both profitable organizations and innovation in the industry. Widely acknowledged as a thought leader, she presents at a range of professional meetings and conferences. Ann maintains an active professional network and affiliations, including membership in the Healthcare Executive Leadership Network and Women Business Leaders of US Healthcare. She serves on the Healthcare Services Strategic Advisory Council for UMB and advisedseveral start-up companies. She has been an active member of the board of Round Earth Media, a non-profit organization that focuses on sustainable business models for journalism. In 2016 she was inducted in the Chicago Entrepreneurship Hall of Fame.

Ann graduated from Carleton College and received both her Masters in Business Administration and Health Care Administration from the University of Minnesota.
