Mary C. Sajdak

Mary Sajdak started her career with CCHHS in 1992, when she served as the Associate Administrator of Ambulatory Care for 3 years, managing the day-to-day operations of Fantus Health Center. After spending nearly two decades in the private health care sector, Ms. Sajdak returned to CCHHS, assuming the role of Senior Director of Integrated Care in 2013. In her role as Senior Director, Ms. Sajdak develops programs and services to support patients and physicians at the health system. Ms. Sajdak is responsible for the health system’s patient support center, referral center, inpatient and outpatient care coordination and managed care for CCHHS. Ms. Sajdak holds a B.S. in nursing from the University of Delaware, and earned an M.S. in community health nursing from Rush University. She was promoted to COO, Integrated Services in March 2017.

Sana Syal

Project Manager

Sana Syal works with hospital and clinic providers to incorporate technical solutions that support a more integrated health care system, primarily for Chicago’s safety net population. She currently manages the system development and on-the-ground implementation of eConsult, Medical Home Network’s web-based communication platform that allows primary care providers to connect with specialists and discuss a patient’s specialty care needs. Sana received her Master’s in Public Health, with a focus on Health Management and Policy, from the University of Michigan. Prior to receiving her graduate degree, Sana led Diabetes Self-Management classes for adults and a youth nutrition program in 3 Chicago Public Schools in the Austin community. Sana continues to work with providers to strengthen workflows and enhance technology that holistically addresses community health.
