Christopher M. Patty, R.Ph., DNP, RN, CPPS

Dr. Christopher Patty has been licensed as a registered nurse since 1984, he holds a Doctor of Nursing Practice from the California State University Fresno. He has a strong affinity and deep respect for pharmacists and for ASHP; is a member of the ASHP CME Committee and participated in the First and Second Annual Great Safety Debates at the 2016 and 2017 Summer Meetings. He is certified as a Professional in Patient Safety. Chris has a background in surgical services nursing, healthcare quality improvement and eight years' experience as a full-time medication safety specialist. Chris has a faculty appointment to Fresno Pacific University where he teaches research and project/thesis development to nurse practitioner students.
Chris has participated in numerous research studies as a principal or co-principal investigator, and has published medication safety and nursing quality studies in peer-reviewed journals as well as authoring book chapters and national training curricula for operating room nurses. He has presented medication safety and quality topics nationally since 2015. His primary research interests are in the area of patient safety, and he is the current Vice Chair of the Kaweah Delta Medical Center IRB. Dr. Patty is a past recipient of the Association of California Nurse Leaders' Excellence in Leadership Award, and he participates in a number of community activities related to healthcare and education. He lives in Visalia, California.

Sarah Stephens, DNP, RN, CPPS, Pharm.D., BCPS, CPPS

Sarah Stephens, PharmD, BCPS, CPPS is the Medication Safety Coordinator for Kaweah Delta Health Care District in Visalia, California. Dr. Stephens’ pharmacy career began at the University of Utah Hospitals and Clinics where she completed a PGY1 general practice residency and PGY2 specialty residency in drug information. The majority of Dr. Stephens' career has focused in acute care adult internal medicine including a number of years in academia at the University of Utah College of Pharmacy. Current initiatives and research interests include reducing medication errors in the areas of anticoagulation and pain management, improving transitions of care with an emphasis on discharge medication reconciliation, and evaluating safety culture. Dr. Stephens is most recently served as the Chair of the ASHP Section of Inpatient Care Practitioners Medication Safety Section Advisory Group and is a current member of the California Hospital Association Medication Safety Committee.

Natasha Nicol, Pharm.D., BCPS, CPPS, Pharm.D., FASHP

Natasha Nicol is Director of Global Patient Safety Affairs for Cardinal Health, Inc.

She received her doctorate of pharmacy degree from the University of Maryland School of Pharmacy. She is faculty for the Institute for Healthcare Communication, a certified Just Culture trainer, and invited professor for the South Carolina and Presbyterian Colleges of Pharmacy.

She is past-President of the South Carolina Society of Health-System Pharmacists. She is a Fellow of the American Society of Health-System Pharmacists and served on the Council on Education and Workforce Development, as well as the House of Delegates. She is Program Chair for the ASHP Medication Safety Collaborative. She was recognized for her work with the ASHP Award for Excellence in Medication-Use Safety and was named Pharmacist and Mentor of the Year by SCSHP.

She is a frequent presenter to professional groups, primarily focusing on safety as it relates to culture, use of technology and development of processes.

Jenna Merandi, Pharm.D., FASHP, Pharm.D., M.S., CPPS

Jenna Merandi is a Medication Safety Officer and serves as the Program Director of the second victim program (YOU Matter Team). She was continuing to see firsthand, how medication errors and even near misses impact staff emotionally. In January 2013, Jenna became the Team Coordinator for the development of a second victim program at her hospital and was then appointed as Program Director. She and her team developed a peer based support program to provide rapid support to individuals after being involved in a medication error, traumatic event, or violent situation. The You Matter team which was piloted in the pharmacy department, has sense spread across the entire inpatient areas and outpatient areas (including all clinics, urgent cares, and non-clinical areas) by end of 2015. Jenna has helped train over 700 employees to becoming peer supporters which have provided support to over 500 employees since initiation of program.

Jenna is involved in medication safety within the pharmacy department and at the system level. She works closely with interdisciplinary teams and front line staff to identify individual and system based interventions to prevent medication errors from occurring. Jenna has a faculty appointment serving as a Clinical Assistant Professor of Practice at The Ohio State University College of Pharmacy. Jenna serves as a course director for Systems Issues with Medication Safety since September 2014.
