Purpose: Currently, Patient Aligned Care Team (PACT) pharmacists document treatment plans and recommendations for scheduled clinic appointments but do not have an avenue to document their clinical work outside of face to face or telephone appointments. Without proper documentation of workload, it is unknown how many overall interventions pharmacists are implementing, the types of disease states managed, and the quantity of population health interventions completed by PACT pharmacists. In order to accurately depict workload and types of interventions made, chart review clinic locations are needed for documentation of medication therapy management and recommendations outside of face to face and telephone appointments.
Methods: Chart review clinic locations were requested to be created on August 13th, 2018 with the intent to have them created and activated on September 10th, 2018. A separate location was requested for all clinic sites staffed with VA Puget Sound Health Care System PACT pharmacists. Four different training sessions were held in September to educate PACT pharmacists how to properly document utilizing the clinic locations. The clinic locations were built and went active on September 10th, 2018. That morning a reminder email was sent out to PACT pharmacists, highlighting the names of the clinic locations and encouraging use.
During the week of October 15th, a one-month follow-up and the quantity of encounters completed under the chart review locations will be analyzed using the Clinical Pharmacy Practice Office (CPPO) encounters cube through Pyramid analytic software. After reviewing the number of encounters, a reminder and/or follow up email regarding the locations will be sent out. The encounters will be reviewed for the types of interventions made and the disease states managed by using them. In April, a final analysis of the chart review clinic locations will be performed before presenting the 6-month follow-up results.
