Purpose: The Veterans Affairs (VA) hospitals have developed models of interdisciplinary teams to optimize treatment and care of hepatitis C virus (HCV) patients. Since March 2016, a clinical pharmacist specialist (CPS), practicing as a mid-level healthcare provider, was incorporated into the HCV clinic at the VA Caribbean Healthcare System in Puerto Rico to manage the pharmacological therapy of HCV patients in addition to evaluating medication consults and providing patient education. The purpose of this project is to evaluate the impact on HCV treatment initiation rate and cure rate after integrating a CPS to this HCV clinic.
Methods: A single-center retrospective chart review of patients managed at the HCV clinic will be performed to compare the treatment initiation rate and cure rate of HCV patients before (January 2014- February 2016) and after (March 2016-April 2018) the integration of a CPS at the clinic. Additional data will be collected from VA internal reports of HCV infected patients’ medication use, clinic utilization, and the PhARMD reminder dialog. The primary outcomes of this project are HCV treatment initiation rate, defined as the percent of patients initiated on treatment with HCV direct acting antivirals, and cure rate, defined as the percent of patients treated who attained a sustained virologic response (SVR). Secondary outcomes measured in this project include: percent of patients with at least one encounter with the CPS, number of encounters done by the CPS, type of interventions during these encounters, percent of face to face (clinic) and telephone appointments, number of infected patients screened for HCV treatment, and number of evaluations made to determine candidacy for HCV treatment. Descriptive analysis will be used to present this data. The institutional review board at the VA Caribbean Health Care System have granted approval for this project.
