Purpose: Pharmacists are involved in preventing, identifying, and solving drug-related problems as well as promoting proper use of medication and overall general health. The integration of the pharmacist into primary care settings has the potential to improve interdisciplinary teamwork and patient care. Decentralization of the pharmacist to other practice clinics will allow face-to-face verbal communication between the pharmacist and other health-care professionals. This service could provide a wide range of interventions including quality use of medications, disease management, improvements in medication adherence, and various others. The objective of this study will be to assess the outcomes and interventions that occur.
Methods: This study has been reviewed by the Choctaw Nation IRB and will target approximately 50 ambulatory care patients that will be seen by the endocrinologist in the Diabetes Wellness Center. This group will be termed “Decentralized Pharmacist Group” or DPG. These patients will be recruited over a period of 8 months (October 2018-May 2018). The control group for data analysis will be an equal number of randomized patients being seen in the endocrinology service without a decentralized pharmacist. Specific aims of this study include diabetes-focused disease state management using objective measures such as hemoglobin A1C, daily log of blood glucose, blood pressure, medication refill rates, and other pertinent measures. Other aims of the study include monitoring for improved adherence rates using mean persistence rate (MPR) and portion of days covered (PDC) and assessing patient and provider satisfaction with subjective surveys. Patients will be seen during normal clinic hours. Visits will be in coordination with the endocrinologist to aid in medication reconciliation, therapeutic recommendations, and other miscellaneous interventions. Patient’s data and survey responses will be collected in a HIPAA compliant fashion.
