An Emerging Model of Care for Promoting Physical Activity in Children with Disability: From Hospital to Clinic to Community

Providing guidance to health professionals on effective strategies for including children with disabilities in existing active living programs is necessary for building greater connectivity between medicine, rehabilitation and community- and school-based physical activity. This effort requires a sustainable model that provides rehabilitation physicians and physical therapists with tangible ways to recommend to caregivers effective strategies for promoting physical activity. A newly renovated outpatient rehabilitation facility has been completed on the Lakeshore Foundation Campus that will provide programs and services to children and youth with congenital and acquired physical disabilities. An interdisciplinary team approach will be used to address healthy lifestyles and transitions from medical rehabilitation to community-based sport, fitness, and recreation. The facility will provide outpatient pediatric rehabilitation medicine clinics, intensive day treatment therapy programs, outpatient therapy services including aquatic therapy, and sport, fitness and recreation programs for children and youth. This presentation will discuss this unique collaboration between Lakeshore Foundation, COA and UAB towards the common goal of promoting physical activity and health among children with physical disabilities. Participants will leave with a set of resources that will provide them with tangible solutions for promoting physical activity in their discharged patients at the local community level.

Jim Rimmer, PhD

Endowed Chair in Health Promotion and Rehabilitation Sciences

Dr. James Rimmer has focused his 35-year career on promoting the health and wellness of children and adults with physical and developmental disabilities. He currently serves as director of the UAB-Lakeshore Foundation Research Collaborative and is also the director of the National Center on Health, Physical Activity and Disability (NCHPAD, www.nchpad.org), which has been funded by CDC since 1999. His vision for promoting the health of children with disabilities is by helping to bridge the gap between the work of physical therapists in the clinical setting with a smooth transition into community-based and home-based exercise, recreation and sports.

Division Director and Associate Professor, UAB Division of Pediatric Rehabilitation Medicine

Drew Davis serves as Medical Director for the UAB Division of Pediatric Rehabilitation Medicine at Children's of Alabama. He is an Associate Professor in the UAB Departments of Pediatrics and Physical Medicine and Rehabilitation (PM&R). Dr. Davis is certified by the American Board of Physical Medicine and Rehabilitation in PM&R, Pediatric Rehabilitation Medicine, and Brain Injury Medicine. Dr. Davis is a member of the American Academy of Physical Medicine and Rehabilitation, the American Congress of Rehabilitation Medicine, the American Academy for Cerebral Palsy and Developmental Medicine, and the American Academy of Pediatrics.

Dr. York is Director of PT&OT Department and Early Intervention Services of Children's of Alabama. She has served in multiple roles for Alabama and the American Physical Therapy Associations, Past-Chair of Hospital-Based SIG of APPTAC, and current President of the International Organisation of Physical Therapists in Paediatrics of the WCPT. She has served as a leader in developing PT&OT services in a community based setting in collaboration with the Lakeshore Foundation.
