Health plans and providers alike struggle to improve patient transitions of care, and to reduce rates of readmission. One well-established method of improving outcomes during these transitions is medication reconciliation post-discharge (MRP). Furthermore, studies have demonstrated that pharmacist-delivered MRPs can deliver significant value, especially in reducing preventable adverse effects and overall healthcare utilization.
How can health plans incorporate MRPs? Is it feasible for health plans to provide medication reconciliation outside of an integrated system? What challenges, and payoffs, might be anticipated from such an initiative?
