RT-2 - Leveraging SNF Rehabilitation Services to do More with Less – SNF Operators and Therapists Share Strategies in How They Have Redesigned Operations to Focus on Managing Outcomes – Not Minutes

The American Health Care Association (AHCA) and National Association for the Support of Long Term Care (NASL) are AOTA Single Course Providers. As Single Course Providers, AHCA and NASL are authorized to offer .4 AOTA CEUs/4 Contact Hours.
There are no prerequisites to attend RT-1 or RT-2 but it should be notes that RT-2 is considered an Intermediate Level Session.


We are facing a healthcare environment where SNF margins are shrinking and payment models are shifting rapidly from reimbursing for services provided to alternative payment models based on quality and outcomes. As SNFs consider options for redesigning clinical operations to be more efficient, cutting costs alone may not be ideal path to success. Instead, a strategic reassessment of all areas of operations that impact the costs of providing care, clinical outcomes achieved, and length-of-stay need to be evaluated to assure that effective resident-centered care remains the focus as payment models change.

This panel discussion will center on how rehabilitation services can be better integrated into SNF admission, care plan implementation, discharge, and follow-up activities to assure that the resident receives optimal care and outcomes. Specifically, the panelists will focus on considerations, approaches, challenges, and results that SNF operators and their rehabilitation partners have achieved in implementing changes to be successful in Medicare Advantage, ACO, BPCI, CJR and other models. The panelists, including a beneficiary representative, will also share examples of how applying these successful strategies has benefited their quality and financial performance and “resident-centeredness” under all payment models. Examples will demonstrate how PT, OT, and SLP personnel contributed to high-volume and high-impact areas where therapist knowledge had been underutilized in the past. Finally, the panelists will provide examples of how using data metrics developed under these models to position themselves better with physicians, hospitals, payers, and consumers.

This session would feature a panel of SNF operators, therapy providers, and a beneficiary. The SNF operators represent a range including large nursing chains, independent owners, regional multi-facility, and non-profit providers.

Garry Pezzano has served as Vice President of Clinical Practice for Genesis Rehab Services since 1997, and was promoted to Senior Vice President in 2011. Under his leadership, the Clinical Practice department has taken a significant role in supporting the company's operations and driving the way clinical care is delivered.
As a licensed speech-language pathologist, Garry brings more than 25 years of clinical and management experience to Genesis. He has held a number of leadership positions with national rehabilitation providers, always maintaining a clear clinical focus. Garry is active in national professional associations, including the American Speech-Language-Hearing Association (ASHA), the New Jersey Speech-Language-Hearing Association (NJSHA), the American Occupational Therapy Association (AOTA), and the American Association for Respiratory Care (AARC).
Garry currently serves as President for the National Association for the Support of Long Term Care (NASL), and formerly served as chair of its Medical Services Committee as well as Board Secretary. He also serves on the Joint Commission task force for ASHA, the American Physical Therapy Association (APTA) Business Partners’ Advisory Board, and is on faculty for the Post-Acute Care Center for Research (PACCR). In addition, Garry is co-founder and co-chair of the Long Term Care Rehabilitation Consortium (LTCRC), a group of clinical leaders from national rehabilitation providers who are committed to improving the standard of rehabilitation services in long-term care.
Garry received a Bachelor of Science in Communication and Speech-Language Pathology from Trenton State College, and a Master of Science in Speech-Language Pathology from Rutgers University. He has also completed Advanced Topics in Healthcare from Harvard University's School of Public Health and Wharton Business School, and is a certified Lean Sigma green belt. Garry is currently enrolled in Rutgers University's Master of Business Administration program.

Robin L. Hillier, CPA, STNA, LNHA, RAC-MT is the co-owner of Lake Pointe Rehabilitation and Nursing Center in Conneaut, Ohio abd Quality Life Services - a home healthy agency in Ohio. She is also President and owner of RLH Consulting, which has provided operational, quality, and reimbursement consulting to long term care providers since 1998. She is a certified public accountant, a state tested nurse aide, a licensed nursing home administrator, and Resident Assessment Coordinator-Certified and a Master Trainer through the American Association of Nurse Assessment Coordinators. She is a member of the AANAC Expert Advisory Panel.

Ms. Hillier has almost 30 years' experience in long term care, including facility operations, quality improvement, MDS, case mix, cost reporting, rate setting review and analysis and reimbursement consulting and representation for skilled nursing facilities and ID/DD providers.

Ms. Hillier is the Independent Owner Representative on the American Health Care Association Board of Governors and serves on the Executive, Business Management, Audit, Finance, ID/DD, and Public Education and Communication Committees. She is a Past President of the Ohio Health Care Association's Board of Directors and served on the board for 16 years. She also previously held the offices of President, Vice Chair, Secretary and Treasurer, and is a past chair the OHCA Payment for Services Committee and the Convention Task Force. She is Past President of the Educational Foundation of the Ohio Health Care Association (EFOHCA) and serves on its' Education Committee. EFOHCA provides scholarships for long term care employees who are advancing their education and provides continuing education to long term care professionals.
