DA-2 - How’d They Do That? A Dataholic’s Guide to Great Performance

Nursing centers face a world gone mad, with payment systems in a continual state of flux and uncertainty, and increased focus on quality and resident experience. The ongoing challenge is to maintain profitability, increase efficiency, and improve the care provided. The presenters have worked collaboratively to identify nursing centers that are achieving excellence clinically and financially, pinpoint the metrics that set them apart from other centers, and implement organizational changes to recreate those results in other facilities. Our speakers bring more than 75 years of collective experience in their respective disciplines to bear on a new, aspirational benchmarking approach, one that identifies and weighs metrics that are indicators of quality care and normalizes financial performance for variability in Medicaid rates and other factors to identify the best-performing nursing centers in the country. The benchmarks, which include staffing standards, billing and coding issues, costs of care, and resident statistics, contrast operational indicators that set the very best facilities apart. Of course, just knowing the benchmarks isn’t enough. Presenters will also discuss experience-based approaches to closing gaps between a center’s current state and desired state and maintaining great performance. This session will engage participants using text-based polling to tailor the discussion to their interests. In addition, participants will collaborate with the presenters to compare data from their own facilities with data from the best performers.

Lara is the Director of Care Coordination for Cantex Continuing Care Network. In her role, Lara oversees the company’s focus on alternative payment model strategies with hospital, physician partnership and value based programming. She also works with all business lines to focus on the company’s Care Transitions Program. She is the corporate champion for lowering readmissions, INTERACT, and disease management training. She is part of the clinical team for the company wide electronic medical record development and implementation. As a super-user she has overseen the program development from a provider perspective and leads the physicians, nurse practitioners and physical assistants training. Her additional responsibilities include overseeing new company initiatives that target the changing health care landscape and is a frequent speaker at local and national venues.

Lara earned her B.S. in Psychology at Texas A&M College Station and her B.S.N. at University of Texas Medical Branch Galveston. She earned her Master’s in Nursing as a family nurse practitioner from the University of Colorado Health Sciences Center in Denver where she was selected as for a Robert Wood Johnson Fellowship. She has additional certifications in MDS assessments and is a certified nurse executive through ANAAC. She is currently an appointed member of American Health Care Association, where she serves on the clinical committee. A mother of two wonderful young adults, she is a life-long learner and patient advocate for quality patient care and the patient experience.

Loretta Kaes is the Director of Quality and Clinical and Regulatory Services for the Healthcare Association of New Jersey. Loretta is a clinical resource for Assisted Living and Long Term Care, Skilled, Nursing Facilities. She oversees the Best Practice Committee, HCANJ’s Top Gun School for Assisted Living Nurses and the TOP GUN School for Long Term Care and Post-Acute Nurses. Loretta is responsible for the training and implementation for the INTERACT Quality Program in Assisted Living Communities as well a Skilled Nursing Centers as well the new RoP requirements..

Previously, Loretta Kaes, was the Vice President of Health Services for Chelsea Senior Living, LLC. In this position, she was responsible for the clinical and nursing operations in sixteen Assisted Living and Residential Care facilities in New Jersey, New York and Pennsylvania. Her responsibilities included Quality Assurance of safe nursing practices and clinical operations to comply with state and federal regulatory rules and guidelines.

Over a 20 year span, Ms. Kaes held various positions with Chelsea, including Executive Director and Regional Director of Operations. Loretta has over thirty-- five years’ experience in long term care as a DON, LNHA and CALA. She is a frequent guest speaker for state and national organizations and is a member of AHCA Clinical Practice and Quality Network Committees as well as NCAL Quality Committee. Loretta serves as a Board Member of the National Executive Board of the American Assisted Living Nurses Association (AALNA) and represents AALNA on the Center for Excellence in Assisted Living (CEAL) Executive Board.

Chris, a member of BKD National Health Care Group, has more than 23 years of experience serving health care providers, including nursing facilities and other senior services providers, hospitals, cognitive-impairment-related organizations, physicians, hospices and home care agencies. While he currently focuses primarily on advanced payment model, operational and Medicare and Medicaid reimbursement consulting services, Chris has experience with financial statement audits, account outsourcing services, Medicare and Medicaid compliance services and Medicaid rate setting. Chris is a member of the American Institute of CPAs, Oklahoma Society of CPAs and Healthcare Financial Management Association. He serves on the Finance and Executive Committees for Family & Children’s Services, a Tulsa-based behavioral health organization. He has presented seminars on Medicare and Medicaid reimbursement topics, continuum of care issues, the Health Insurance Portability and Accountability Act and other related areas for LeadingAge Oklahoma, Oklahoma Association of Health Care Providers, Arkansas Health Care Association and other state and national health care professional associations. Chris is a 1991 graduate of The University of Oklahoma, Norman, with a B.B.A. degree in accounting, and a 1993 graduate of Oklahoma State University, Stillwater, with an M.S. degree in accounting.
