PR69 - Treatment of Mature Teeth With Irreversible Pulpitis: A Long Forgotten Vital Pulp Therapy

Objectives: The purpose of this pilot clinical trial is to evaluate the feasibility of Vital Pulp Therapy (VPT) of mature permanent teeth with irreversible pulpitis using Augmentin as antimicrobial agent. Materials and Methods: Mature teeth diagnosed with irreversible pulpitis that had carious vital pulp exposure upon excavation were included in this study. Under local anesthesia and rubber dam isolation, all caries were removed and the exposed infected pulp was surgically amputated until fresh healthy pulp was reached with a high-speed handpiece under sterile saline solution. Bleeding was controlled with copious amounts of 3.25% sodium hypochlorite irrigation and application of sterile cotton pellet. The pulp wound was dressed with Augmentin paste and access cavity was closed with sterile cotton pellet and glass ionomer cement for two weeks. The teeth were re-accessed under local anesthesia and rubber dam isolation. Augmentin paste was removed with copious amount of sodium hypochlorite irrigation and saline solution. The pulp wound was then covered with Biodentine and restored with composite resin. The treated teeth were followed up at 3, 6 and 12 months. Results: All 20 mature teeth with irreversible pulpitis treated with VPT were asymptomatic and responded to pulp sensibility tests at 12 month follow-up. There was no radiographic evidence of internal resorption or periapical lesion. Conclusions: Augmentin appeared to be an effective antimicrobial agent and Biodentine provided good sealing ability. Based on the limitations of this pilot clinical study, VPT of mature teeth with irreversible pulpitis appears to be a feasible alternative treatment option to root canal therapy.
