PR78 - Use of an Evidenced-based Surgical Algorithm for the Retrospective Analysis of Root-End Surgery in a Graduate Endodontic Practice

Introduction: Evidence-based algorithms and checklists serve to guide clinicians’ decision-making. An ideal algorithm is accurate, concise and reflects current best evidence. No such algorithm exists to guide endodontic microsurgery. The purpose of this study was to develop an evidence-based root-end surgical algorithm and to retrospectively test its validity and utility. Methods: An evidence based root-end surgical algorithm was designed. From the EDGES Algorithm (Evidence-based, Decision-making, Guidelines for Endodontic Surgery), 13 critical decision points were extracted to create the EDGES Surgical Checklist. A retrospective chart review was conducted of 267 root-end surgeries performed in the OHSU Graduate Endodontic Clinic from February 2010 - November 2016. Deviation at any critical decision point was identified, classified, and evaluated. Results: The algorithm critical decision points were validated based on best evidence. In 11 of 13 critical decision points, surgeries conformed to evidence-based guidelines. Deviation from the algorithm was minimal. In 2 of 13 points, there was poor conformity to guidelines; opioids were routinely prescribed in 48% of cases and post-treatment evaluations at ≥6 months were completed in 39% of cases. Deviation was determined to be a lack of an evidence-based decision by the provider. Conclusion: The EDGES documents proved to be useful and valid tools to guide evidenced-based surgical decision making. The EDGES Algorithm could be a valuable resource to those new to endodontic root-end surgery. Root-end surgeries performed at OHSU generally conformed to evidence-based guidelines.

Dr. Gordon Marshall was awarded his DMD in 1979 by U of Oregon Health Sciences Center. He received a certificate of Anesthesiology in 1980 from OHSU Medical Residency. He completed his certificate in Endodontology in 1983 in Medical College of Georgia. Dr. Marshall was the first recipient of the AAE part time educator's award. He has served for over thirty years as a faculty in the Department of Endodontology at OHSU.

Christine Sedgley

Dr. Sedgley did her dental training at the University of Sydney, Australia and her endodontic training at the University of Melbourne, Australia. She received her PhD in Microbiology from the University of Hong Kong. She is currently the Department of Endodontology Chair at OHSU. Go Blue!
