: Overview of an effective infection prevention and control program using QAPI to include CMS elements of QAPI, infection prevention systems compliance, and appropriate elements (surveillance, antibiotic stewardship, observation, and reporting relationship to QAA/QAPI). Also touches on QAPI integration, PIP, and ad hoc examples, and provides greater details on surveillance, AB stewardship, and observation tracking and trending. Includes discussion of facility assessment with integration of annual infection prevention risk assessment. Additional elements include policy and procedure elements to consider and how to communicate priorities to residents, staff, and families. Stacey Merritt Hord is a health care executive and operator with expertise in operational systems compliance through QAPI as aligned with Baldrige as Business Management System.

Stacey is the 2016/2017 Chair of the AHCA/NCAL Quality Improvement Committee andSenior Vice President of Clinical Operations for Sava SeniorCare.

She was also elected to serve as Board member of the 2016/2017/2018 AHCA/NCAL National Quality Award Board and serves as member of the 2016/2017 AHCA Quality Cabinet, 2016/2017 NCAL Quality Committee and 2016 PHCA Quality Committee.

Facilities under her operational oversight as an LNHA received AHCA Bronze and Silver recognition along with Alabama Level 1 Commitment to Excellence, Level 2 Progress Toward Excellence, and Level 3 Alabama Excellence recognition.

Her faculty experience includes the AHCA/NCAL national convention, Alabama Quality Award Conference, AHCA Quality Symposium/Summit, various state conferences/associations, and other venues.

Stacey served on the panel of experts for the AL Quality Award Conference from 2004-2008 and has served as an AHCA Quality Award Examiner in multiple roles (Bronze, Silver, Silver Team Leader, Master Examiner, Master Examiner Team Leader) since 2002.

She also served as a member of the AHCA Quality Improvement Committee (QIC) and AHCA QAPI subcommittee from 2012- 2015. In addition, Stacey served as Chair of the 2015 QAPI Tools and Resources WorkGroup for the Quality Improvement Committee where her team developed and recorded QAPI content for AHCA/NCAL's Learning Management System, ahcancalED.

She has been a member of the AHCA Public Education Campaign (PEC) since 2012 and served on the 2013 and 2016 AHCA Professional Workgroup Development Committees. Stacey was also a member of the 2012 CMS Antipsychotic Quality Measure Technical Expert Panel (APQM TEP) and served on the 2014 CMS Person Centered Care Technical Expert Panel (PCC TEP).
