(EP-147) Telemedicine Program ORTIF in the Biggest French Region: Highly Versatile Tools to Allow Multiple Uses

Background :
The telehealth platform ORTIF has been implemented since January 2015, in one of the largest administrative area of France and concerns 12 M inhabitants. ORTIF Platform enables by way of a digital interface on the web net, to share health and care contextual datas, to transfer native radiology image data, and to organize real time medical consultations by means of videoconference between multiple actors from multiple remote locations. It currently involves 95 experts and demanding health facilities. It covers the whole field of emergency neurology (neurosurgery, stroke, exacerbation of chronicle neurological disease, preterm newborn neurologic expertise), teleradiology, dermatology, autism care, child with multiple disabilities care and extension to all medical specialties.

Methods.
Considering the multiplicity of actors and medical specialties involved, the regional group SESAN, that pools together health facilities, medical and social structures, prison facilities and primary care, carries out the project, including designing medical procedures and organisational workflows, implementation, assistance and mentoring. A scientific committee has already defined the content of specialized forms and versatile forms to exchange medical electronic datas. An infrastructure for a secure, cooperative, asynchronous and synchronous teleradiology system was deployed. Facilitating access to videoconferencing tools was identified as a priority objective and should allow the sharing of screens for collaborative exchanges.
The impact analysis of the implementation of these actions has been carried out over the first 18 months of operation, between january 2015 and July 2016, with the 95 establishments involved.

Results-discussion
After 18 months of use, 27806 patients were managed with the telehealth Platform ORTIF (9732 during the last 3 months) : 8866 patients for neurosurgical expertise (currently monthly mean-CMM :541), 608 for stroke expertise (CMM:55), 12790 with using versatile forms in all medical specialties (CMM:1673), 5103 for teleradiology expertise (since January 2016)(CMM :502), 205 for dermatology expertise (since January 2016)(CMM :70), 73 for preterm newborn neurologic expertise (since January 2016)(CMM :12), 140 for child with multiple disabilities care expertise (CMM :20), 21 for autism expertise (CMM :5).
ORTIF currently hosts the largest number of monthly uses of telemedicine in France.
A former experience in telemedicine could explain the numerous uses for neurosurgical expertise and teleradiology. Accessibility, simplicity and versatility forms and tools promote though new uses in all medical specialties and launch the process of change in certain pilot sites or fields.

Conclusions
Telemedicine should become an indispensable support of all medical organizations to allow better access and higher quality of the care. The professionals should also perceive these new tools as bringing in real assistance rather than constraints. Telemedicine efficiency relies on the consistency and the versatility and complementary nature of the tools implemented.
