(EP-151) Five-Year Progress of Technological Changes in Remote Medical Education

Objectives
Telemedicine is of great help for the education of young doctors, providing latest knowledge and easy access to specialists. However, it is often difficult for doctors to use it, because technological infrastructure, such as networks, systems, and devices, is limited in many hospitals. Since 2003, our research group has proactively worked to advance international remote medical education implemented through the Internet. This paper presents analysis of our recent 5-year activities to evaluate technical transition and the effects on styles of remote medical education.

Methods
We analyzed remote medical education programs that we performed from 2011 to 2015; 344 programs with 346 institutions in 51 countries, and connected 1,969 sites in total. We used a digital video transport system (DVTS), H.323, and Vidyo videoconferencing, along with a commercial network or research and education network (REN) installed in the subject hospitals. Questionnaires were randomly distributed among 2,045, 1,573, 1,853, and 1,947 viewers for feedback evaluating image resolution, image movement, sound quality, and programs, respectively.

Results
The annual number of programs was increased to over 90 per year in 2014–2015. Although 79% (38/49) of all programs took place at less than four sites in 2011, 54% (49/90) of programs were held at over five sites in 2015. Over the course of the study, the main system was changed from DVTS (58%, 30/52 in 2011) to Vidyo (64%, 67/104 in 2015). Within the 273 newly connected institutions, 149 were connected in 2014–2015; more than double the 70 in 2011–2012. University institutions decreased compared with non-university institutions, which increased to 61% (39/66) in 2015. Asian institutions accounted for the largest share (74%, 201/273), followed by Latin American (13%, 35/273). Image resolution and movement recorded over 92% in positive ratings. Sound quality was somewhat lower, at 83%. Ultimately, 98% of respondents called the programs “very good” or “good.”

Conclusion
Continuous technical development was observed with increasing sites for each program. This also resulted in expansion of activity involving non-university settings and small hospitals. The quality provided by the image interface and audio was preserved. Hence, practical remote medical education, which satisfies doctors’ need for quality and inclusiveness, could be expanded to hospitals around the world.

Kuriko Kudo, Ph.D., has worked at the Telemedicine Development Center of Asia (TEMDEC) at Kyushu University Hospital since August 2011. She graduated from Kyushu University with a doctorate in design, with particular emphasis on digital archives using 3D computer graphics based on physical measuring and modeling that account for motion, shape, lighting, and the spectral reflectance of materials.
She currently works in technical support for remote medical education as a research assistant professor and telemedicine engineer. She is involved in engineering training as a Steering Committee member of the Asia Pacific Advanced Network (APAN) Medical Working Group. She also leads research on communication design for handling international medical teleconferences and development of online database systems.

Shuji Shimizu

Dr Shuji Shimizu, M.D. Ph.D. is a surgeon, having graduated from Kyushu University, Fukuoka, Japan. He is the Director of the Department of Endoscopic Diagnostics and Therapeutics, and majors in endoscopic surgery. Dr Shimizu is currently the Chairman and Professor of the Department of International Medical Department and is the Director of the Telemedicine Development Centre of Asia, where for more than a decade he has facilitated a comprehensive telemedicine project between Korea and Japan. This project has widely spread in various Asia-Pacific regions and is expanding globally, collaborating with 445 institutions in 57 countries/regions and having organized over 600 programs. He has invited more than 200 young doctors and technical staff from all over the world for medical and engineering training as the Director of Overseas Exchange Center. Dr Shimizu organized the Medical Working Group of the Asia-Pacific Advanced Network (APAN) in 2005 and has been chairman over a decade. He also started Asia Telemedicine Symposium in 2007 where both doctors and engineers gather to share the information and to find out needs and problems in common. He is a Training Centre Committee member of the World Gastroenterology Organization, serving as a driving force for telemedicine to connect worldwide centers for continuous medical education. He newly won a 5-year grant in April 2016 from the Japan Society for the Promotion of Science focusing on Latin America to facilitate telemedicine for the early detection of gastric cancer, where the collaboration with RedClara is highly expected.
