(EP-105) Characterization of Medical Consultation Addressed to Pulmonology Through Asyncronous Telemedicine in Colombia

Objectives
The Teleconcept is a modality of asynchronous telemedicine used in Colombia, which allows a specialized doctor to give an answer with his specialized medical concept during a determined period of time. Actually in Colombia there aren´t any data base information about the characteristics of the teleconcepts directed to the Pneumology service.

Methods
A cross-sectional study was performed in order to identify the characteristic of the teleconcept directed to Pneumology service. The information was obtained from the clinic history of the requested teleconcept, referring to the date and hour of the request, vital signs, CIE-10 diagnosis, main chief complaint, management of the teleconcept by the Pneumologist, date and hour of the teleconcept answer given by the specialist. The sample size was calculated for a proportion 50%, precision 5% requiring a minimum of 462 teleconcepts, data analysis was done through descriptive statistics, quantitative variables were summarized in medians and interquartile ranges and qualitative variables in frecuencies and percentages.

Results
766 teleconcepts were analyzed, the median age 60 years, 55,5% women, in more than 80% the vital signs were reported, pulmonary auscultation 19,5%; the most frequent symptom was diypnea 47%, cough 44,1 %, fever 27%, frequency the medical history antecedents were chronic obstructive pulmonary disease (COPD) 31,6%, smoking 15,9 %, asthma 11,4%, obstructive sleep apnea 8,2%, tuberculosis 7,4%. The diagnosis based on the CIE-10 clasification: COPD 19,1%, asthma 6,9%; test reported: chest x-ray 46,5%, flow-volume curve spirometry 31,5%. The request for general evaluation of pulmonology was for 58,5%, treatment orientation 31,3%, diagnosis aproach 15,7%, radiologic findings interpretation 11,1%, abnormal paraclinical findings 9,7%, definition of procedures 0,8%, and forecast 0,15%. Average response time for the teleconcept was 9,1 hours. The conduct that was ordered by the pneumologist was medical monitoring or follow up 78,5%, performance of diagnostic test 72,8%, medical treatment 59,7%, and face to face evaluation by the pneumologist 31,5%.

Conclusion
In this study it was found that the medical history data reported more frequently associated with pulmonary pathology was COPD. Cough and dyspnea was the most frequently main chief complaint reported by the general practitioner,who in most cases apply for the general support from the pneumologist. The specialist decision was to performed medical monitoring or follow up, paraclinical request, and medical management of the patient case.. The assessment diagnostic test most frequently requested was the flow-curve volume spirometry.
