3200 - IV to PO to Infection Prevention : What’s the Infection Preventionist’s Role in Antimicrobial Stewardship?

Infection prevention was deeply involved in building an antimicrobial stewardship program at Providence Saint John’s Health Center and played an instrumental role in helping accomplish the antimicrobial stewardship program’s goals. The antimicrobial stewardship program has been active since June 2013. Early-stage goals (July 2013–March 2014) included educating medical staff on the antimicrobial stewardship program and developing regular antimicrobial stewardship program data reports to be presented to the infection prevention committee. The following interventions were tracked to establish quantifiable antimicrobial stewardship program interventions: IV to PO, dose optimization, redundant therapy, general de-escalations, targeted de-escalations of daptomycin and ertapenem, acceptance rates (88.5 percent), and rejection rates (11.8 percent). Late-stage goals (April 2014–December 2015) included creating an antimicrobial stewardship program policy with clear guidelines to hold physicians accountable, improving data tracked and reported to the infection prevention committee, MD/ID percentage noncompliance, developing hospital-specific antimicrobial guidelines, assisting antimicrobial stewardship program with educating noncompliant physicians, and championing for a full-time antimicrobial stewardship program pharmacist. The following interventions were tracked: acceptance rates (93 percent), rejection rates (7 percent), and duration of therapy/1000 patient days (for Clostridium difficile and non-C. difficile). Together, infection prevention and the antimicrobial stewardship program created awareness of the antimicrobial stewardship program through regular reporting to the infection prevention committee and by creating antimicrobial stewardship program guidelines, which became policy in 2015. Initially data focused on cost savings, thereby gathering further funding. Advanced-stage data emphasized the clinical benefits of the antimicrobial stewardship program. Infection prevention brought together noncompliant physicians and assisted the antimicrobial stewardship program in holding them accountable. At inception of the antimicrobial stewardship program, there was only one part-time antimicrobial stewardship program position; by June 2016, the position became full time. Infection prevention and the antimicrobial stewardship program exhibited strong collaboration by achieving early- and late-stage goals through a team effort. By accomplishing these goals together, infection prevention and the antimicrobial stewardship program hope to create long-term and sustained decreases in healthcare-associated infections.

Dr. Debesh Das is an Infection Preventionist at Providence Saint John’s Health Center, in Santa Monica, California. Debesh has worked as an Infection Preventionist since 2012, in various hospitals within the Los Angeles area during that time.

Debesh graduated in 2000 from the University of California, Irvine with a Bachelor’s of Science in Biology and a Minor in Religious Studies. He then matriculated in Medical School at Ross University School of Medicine in the island of Dominica with training in New York area hospitals and graduated with a Medical Degree in 2007. Most recently, in 2012, Debesh obtained a Masters in Public Health, with a concentration in Medical Epidemiology from Loma Linda University, in Loma Linda, California.

Debesh is the 2018 President Elect for the Greater Los Angeles Area APIC chapter and former Membership Chair for the chapter.

Snezana Naumovski

Infectious Disease/Antimicrobial Stewardship Clinical Pharmacist

Born in Chicago into an immigrant family, Snezana was raised in Macedonia, where she graduated from Saints Cyril and Methodius University of Skopje Pharmacy School with her PharmD degree in 2003. Based on her interest in GMP and pharmaceutical manufacturing, she worked as a manager in the development of a major pharmaceutical manufacturing facility. She returned to the United States and earned her pharmacist license in 2009, focusing on areas of clinical pharmacy in inpatient care settings and acute care hospitals. She was certified through SIDP as an antimicrobial stewardship pharmacist in 2013. Snezana has worked on implementing and developing an antimicrobial stewardship program at Providence St. John's Health Center, managing outliers and noncompliant medical staff with recommendations and by establishing policies, procedures, and guidelines to fight antimicrobial resistance. In 2015, she took a pharmacy student preceptor role as an adjunct professor at the University of Southern California Pharmacy School.

Angela Vassallo is an Infection Prevention Specialist with Health Services Advisory Group, Inc. In this role she is leading a groundbreaking CMS funded project to reduce SSIs in California Ambulatory Surgery Centers. Prior to working with HSAG, she was Director of Infection Prevention/Epidemiology at Providence Saint John’s Health Center in Santa Monica, CA. She is Faculty at West Coast University in Los Angeles, CA and Providence University in Great Falls, Montana. She helped create the first Masters program in Infection Prevention at Providence University in 2016. Angela is certified in Infection Prevention and Control with a “CIC” designation and is an APIC Fellow with the “FAPIC” designation. Angela is Past-President of CA APIC and Greater Los Angeles APIC. She is Vice-Chair of the national APIC Communications Committee, a member of the BD Speakers’ Bureau for reducing surgical site infections, and a founding member of Expert Stewardship, Inc.
