Tailoring a CBT Implementation Protocol Using Mixed Methods and Conjoint Analysis

Tailored implementation approaches are touted as more likely to support the integration of evidence-based practices. However, to our knowledge, no exemplars exist for tailoring implementations in behavioral health. This study is the first to tailor a cognitive behavioral therapy (CBT) implementation protocol using a model-based, mixed methods prospective needs assessment to identify contextual barriers paired with a modified conjoint analysis. Mental health clinicians and operations staff at two sites of an adolescent residential treatment center completed surveys (clinician N=21; operations staff N=49) and participated in focus groups (clinician N=15; operations staff N=38) guided by the domains of the Framework for Diffusion (Mendel et al., 2008). Analyses revealed 76 unique barriers, which agency administrators then prioritized using rating scales of feasibility (high/low) and importance (high/low). Twenty-three prioritized barriers were subjected to a conjoint analysis wherein administrators and implementation team members selected implementation strategies. Implementation researchers then rated strategies based on feasibility (high/low) and potential impact on CBT adherence (0=none to 3=very high impact). Top-rated strategies (N=36 discrete strategies) were then matched with the barriers and compiled into a three-phase implementation blueprint. Implementation teams led strategy enactment prior to CBT implementation. The needs assessment surveys were re-administered to clinicians and staff (N=49) at one year follow up.
